Explanation of * items:
1. Compensation to be divided into ten equal monthly
installments, beginning on the second positive beta and when the
doctor feels there is not a need for another beta, or upon verification of fetal heartbeat ultrasound, then compensation
will begin. Compensation will be paid every 4 weeks thereafter.
Experienced surrogates may request $25,000 to $40,000.
2. Multiples fee ($5,000; each additional fetus $4,000).
3. Transfers and Inseminations (whether IUI or HI and per month) are both $500 per cycle.
4. Mock cycle with medications, and cancelled or dropped cycles (due to no fault of the surrogate) are reimbursed at $250 per cycle.
5. Invasive procedures (each procedure is separate, regardless if they are performed on the same day) are to include amniocentesis (per sac), cerclage, CVS or ectopic pregnancy with or without tubal removal, DNC undergoes or experiences a miscarriage with a clinical abortion), HSG, SIS, selective reduction, hysteroscopy, termination, abortion
prior to 12 weeks are $750. Abortion after 12 weeks $1,000. Post-partum Invasive procedures relating to the pregnancy and/or delivery are included up to eight (8) weeks after delivery or termination of pregnancy.
6. Maternity clothing allowance is payable upon reaching 12 weeks
gestation. Additional $250 for multiples due at the beginning of
24 weeks gestation.
7. Maternity expense allowance is a non-accountable monthly expense beginning
from the signing of contract to 2 months after delivery. In the event of multiples, the allowance is $300.
8. Cesarean sections are reimbursed at $3,000. This is aside from any bed rest or lost wages.
9. The loss of any female reproductive organs is $5,000.
10. Life insurance in the face value policy of $350,000 (300,000
directed toward the Surrogate's family and 50,000 directed toward
the Intended Parents).
11. Housekeeping allowance is a weekly allowance, $50 every week, beginning on the 1st day of the third trimester of pregnancy and ending 4 weeks after delivery of a child.
12. ALL expenses related to travel for the medical/psychological screening, cycling, transfer, pre-natal, delivery appointments are to be covered by the Intended Parents in whole. This includes ALL mileage (in excess of 50 miles round-trip) , lodging, meals, and childcare expenses.
13. Any and all expenses in regard to relocation will be covered in whole by the Intended Parents.
PLEASE NOTE:
Hospital/bed rest/lost wages in the event of doctor prescribed
bed rest paid 100% as incurred less disability (if available). In
the event that surrogate must take off work to complete ANY appointments
due to the surrogacy, her lost wages will be reimbursed by Intended
Parents. Lost wages for these appointments will be paid in full,
and not covered by disability and/or sick leave.
Lost wages paid for screening/matching (surrogate and companion), transfer (surrogate and companion), delivery/birth (3 days only for companion) and any required legal proceedings (surrogate and husband/partner IF required).
Ob-gyn and hospital/delivery costs vary by hospital/doctor (most
common is the 80/20 plan) and are the responsibility of the Intended Parent.
